- Your University

- The name of your club

- Main contact (does not need to be the captain, but someone who will be attending)

- Main contact's mobile phone number

- Main contact's email address

- The number of players you will be bringing

- The number of teams you will be bringing (if more than 1)

- If you require accommodation Fri/Sat/both

- How you will be getting to the tournament (Car/minibus/coach/train/etc)
